gallstone(cholelith) is a calculus (stone) formed within the gallbladder as a concretion of bile components.
What is Gallstone?
- A gallstone, also called a cholelith, is a stone formed within the gallbladder as a concretion of bile components.
- Gallstones are formed in the gallbladder but may pass distally into other parts of the biliary tract such as the cystic duct, common bile duct, pancreatic duct or the ampulla of Vater.
- Rarely, in cases of severe inflammation, gallstones may erode through the gallbladder into adherent bowel
potentially causing an obstruction termed gallstone ileus. - Gallstones in a cut gall bladder
What it does?
- Presence of gallstones in the gallbladder may lead to acute cholecystitis.
- Presence of gallstones in other parts of the biliary tract can cause obstruction of the bile ducts, which can lead to serious conditions such as ascending cholangitis or pancreatitis.
- Either of these two conditions can be life-threatening and are therefore considered to be medical emergencies.
- Opened gall bladder containing numerous gallstones
Causes
- There can be many factors which can cause gallstones. Here we have listed some possible causes:
- Excess bilirubin in the bile.
- Excess cholesterol in the bile.
- A non-stone-related blockage in the gallbladder that prevents proper emptying.
- Low bile concentration of a substance called bile salts.
Composition
- Gallstones can vary in size and shape from as small as a grain of sand to as large as a golf ball.
- The gallbladder may contain a single large stone or many smaller ones.
- Ultrasound test is done which uses sound waves to visualize the area.
- Another frequently used test is an Oral Cholecysto-Gram (OCG), which uses X-rays to show the gallbladder after the patient swallows pills containing a dye.
- Sometimes gallstones show up as an incidental finding on other tests, such as a CT scan.
- Gallstone risk increases for females (especially before menopause) and for people near or above 40 years.
- A lack of melatonin could significantly contribute to gallbladder stones.
- Gallstones may be caused by a combination of factors, including inherited body chemistry, body weight, gallbladder motility (movement), and low calorie diet.
- A clear relationship has been proven between diet and gallstone formation: non-vegetarians have 9 times the incidence of gallstones compared to vegetarians.

On the basis of their composition, gallstones can be divided into three types:
Cholesterol stones
May vary from light yellow to dark green or brown or chalk white and are oval, usually solitary, between 2 and 3 cm long, each often having a tiny, dark, central spot. They must be at least 80% cholesterol by weight to be known as Cholesterol Stones
Biliruben stones
These stones are small, dark & usually numerous. They are comprised primarily of bilirubin and calcium salts that are found in bile. They contain less than 20% of cholesterol.
Mixed stones
Mixed stones typically contain 20–80% cholesterol. Because of their calcium content, they are often radiographically visible.
Signs and symptoms
Gallstones may be asymptomatic, even for years. These gallstones are called “silent stones” and do not require treatment.
A characteristic symptom of gallstones is a “gallstone attack”, in which a person may experience intense pain in the upper-right side of the abdomen, often accompanied by nausea and vomiting, that steadily increases for approximately 30 minutes to several hours.
